Old fashioned data breach: Independence Blue Cross paper records tossed in trash

Philadelphia Inquirer - (Pennsylvania; New Jersey) Independence Blue Cross in Philadelphia announced December 26 that the personal and health information of 12,500 patients was breached following an October incident where maintenance workers threw out 4 boxes of member records that were meant to be moved to a different floor within the facility.
