Computer hardware containing patient data stolen from Ohio plastic surgery office

SC Magazine - (Ohio) The Beachwood-Westlake Plastic Surgery and Medical Spa in Ohio notified 6,141 patients that their personal information was on a piece of computer hardware that was stolen in a June 29 burglary. The hardware contained names and some medical information.
