Indian Health Services addresses breach of private information

Rapid City Journal - (South Dakota) Indian Health Service Rosebud Service Unit notified 620 patients July 16 after a folder containing their personal information, including Social Security numbers and U.S. Department of Veterans Affairs enrollment information, was quickly recovered after it was accidentally left in a public area of the facility's Rapid City unit May 30 by an employee.
